He was predeceased by: his parents, Alvah D. Cook Sr. and Lucie Lasda Cook; his brother Andy Cook; and his nephew Brian Cook. He is survived by: his daughter De-Anne Cook; his grandson Angelo Cook; and his siblings, Lenny Cook, Linda Frazier and Joanne Bailey. He is also survived by nieces and nephews.

Visitation will be held on Saturday, March 14th 2026 from 1:00 PM to 3:00 PM at the Tully United Community Church (5872 Meeting House Rd, Tully, NY 13159). A funeral service will be held on Saturday, March 14th 2026 at 3:15 PM at the same location.
